Discord staff application template you can actually use

A copy-ready Discord staff application template: gates, availability, prior staff, scenarios, integrity, and what to tell applicants about review.

Four blocks. Delete any you will not score.

Paste this structure into your form builder, or generate a longer version with the Discord staff application generator. Replace the bracketed notes. Delete any section you will not score.

Intro (shown before questions)

  • You are applying for [helper / trial mod / moderator / admin] on [server].
  • Staff will see your answers. We may take [X] days. Silence is a no.
  • Approval may be a trial role first, not full permissions.
  • Discord login is required so we can see account age and username.

Gates (settings, not questions)

  • Must sign in with Discord.
  • Minimum account age: 30 days (helpers) or 90 days (admins).
  • Must already be a member of the server.
  • One open application at a time.

Availability

  1. Timezone and the hours you can actually take pings in a normal week.
  2. Can you sit a defined on-duty window, or only when you happen to be online?
  3. Voice: required, optional, or never for this role.

Prior staff

  1. Have you been Discord staff elsewhere? Server name and a tag we could message.
  2. Why did you leave?
  3. Have you been demoted, blacklisted, or banned from a staff team? What happened.

Scenarios (pick three for helpers, five for mods)

A scenario is an incident, not a quiz.
  • A member is spamming memes in a ticket channel. What do you do in the next five minutes, and what do you write down.
  • A moderator you like is leaking ticket details in public staff chat.
  • Someone reports a friend of yours for harassment. You have DMs with both people.
  • It is 1am. Three accounts are joining and posting invite links. Who do you ping.
  • A new helper used a slur “as a joke” in general. They are popular.

Integrity

  1. Would you recuse yourself from a ticket involving a friend? When would you not.
  2. Have you shared screenshots of staff channels or user reports before.
  3. Anything we will find if we look at your public servers and ban lists.

Close

One optional “anything else” box. Do not add a second motivation essay. Generate the full question set with reviewer notes in the free tool if you want the longer bank.

Copy is fine for a one-off. A queue, Discord login, and a bot that assigns the role is how you stop losing applications in staff chat.
