What a whitelist form should collect

Discord account (login, not a typed tag).
In-game name or character name — one field, not three aliases.
How they found the server, in a sentence.
A short lore / build / playstyle sample if that is actually how you accept people.
Age only if the community is 18+ and you will act on it.

Do not ask for real names, phone numbers, or screenshots of other servers “as proof.” You will leak them. The whitelist guide covers what to hide from public channels.
